The Health and Wellbeing Passport’s primary use is as a document which contains information about a patient that helps health and social care professionals get to know them and their needs.  More than just a medical history and list of medications, the Passport aims to deliver background details giving an insight into the character and personality of the holder. There can be variations on the theme and the Health and Wellbeing Passport can be adapted to suit different circumstances and needs. The aim of the Passport is to quickly establish grounds for rapport between patient and professional and deliver an insight into the holder’s health journey.
