CC+ will deliver an enhanced model of community referral (social prescribing) in Craigmillar to improve the health, wellbeing & life chances of local people.

CC+ will integrate evidence-based psychological training from our involvement in the NES pilot to enhance the team’s work with individuals, as well as developing our community catalyst approach to build local assets and capacity.

Our Link Workers will meet participants on a 1-to-1 basis to build relationships & help them identify key issues, working out together how each person would like to address these.

Using person-centred planning skills to develop an action plan, 1-to-1 support to attend relevant services or groups, & community development work to help establish new activities where gaps have been identified by local people, we will enable people to build individual resilience, wellbeing & community cohesion across the area.
