Supporting self-management in chronic pain: a collaborative approach providing relevant tools to healthcare professionals and people with pain

We will pilot an intervention to overcome barriers to people with chronic pain getting support with self-management from primary care. The intervention will combine two approaches – training for primary care healthcare professionals and a navigator tool to be used during a consultation. Training will be developed in collaboration with the Thistle Foundation and will centre on exploring self-management. The navigator tool, an interactive paper document has 4 components. The first two, ‘My Concerns’ and ‘Questions to ask’ supports the patient in planning their consultation, while an ‘Option Grid’ and ‘Goal Setting’ are for both the professional and patient helping them to plan a self-management strategy together. The intervention will be piloted at two GP practices already identified and 2 further primary care sites.
