Pilot project to explore the impact of pharmacy technician support undertaking home visits to patients and carers following a hospital admission

Discharge from hospital is a process and not an isolated event. There is well established research around the experience of carers around discharge with much of it suggesting that many carers feel unsupported in this process with carer problems contributing to patients’ unplanned re-admissions. Research has highlighted that poor medicine compliance leads to readmission to hospital and that many of these are preventable. Potential harm to patients could be addressed by combining the role of a pharmacy technician within the Carer Support Service to provide guidance and advice on both pharmaceutical issues and carer support needs following hospital discharge.
