Is the Patient Dying? Ensuring Safe, Effective and Person-centred Care

This proposal focuses on improving the quality and safety of care within Lothian acute hospitals, for patients where their recovery is uncertain and they are at risk of dying. The project would test and refine an approach which supports clinical assessment and the delivery of safe and appropriate care for patients and their families, during the transition to either recovery or to end-of-life care. This involves integrating palliative and end of life care within the Scottish Patient Safety Programme- Deteriorating Patient workstream and NHS Lothian quality and governance processes.
