Creative Pathways: Supporting transitions into the Community

The transition from inpatient psychiatric care to care in the community is often experienced as difficult and extremely disruptive to both personal and clinical recovery. It is also the time of highest clinical risk. Continuity of support and structure is associated with smoother and less stressing discharge from hospital, yet for those experiencing psychosis or mental disorganisation there are few accessible resources or relationships to provide continuity from hospital to community. The possibilities of Music Therapy to provide such a support in this transition is to be explored by a music therapist by delivering an open group and individual sessions on demand, in a third-sector community space for residents of Edinburgh.
